(CNN) — Nearly half the respondents in a survey of U.S. primary care physicians said that they would seriously consider getting out of the medical business within the next three years if they had an alternative.

The survey, released this week by the Physicians’ Foundation, which promotes better doctor-patient relationships, sought to find the reasons for an identified exodus among family doctors and internists, widely known as the backbone of the health industry.

A U.S. shortage of 35,000 to 40,000 primary care physicians by 2025 was predicted at last week’s American Medical Association annual meeting.

In the survey, the foundation sent questionnaires to more than 270,000 primary care doctors and more than 50,000 specialists nationwide.

Of the 12,000 respondents, 49 percent said they’d consider leaving medicine. Many said they are overwhelmed with their practices, not because they have too many patients, but because there’s too much red tape generated from insurance companies and government agencies.

Medicaid cuts leave no choice, says doctors, hospitals

Budget cuts in the state’s Medicaid program are forcing a major shift in where Nevada’s poor can seek health care.

Cancer patients who had received outpatient treatment at University Medical Center, for instance, will have to seek treatment at other hospitals and clinics because UMC, citing reductions in Medicaid payments, says it can no longer afford to offer cancer treatment.

Low-income children with bone and spine problems may need to leave Las Vegas altogether for treatment, because pediatric orthopedists are no longer accepting payment from Medicaid because of cutbacks to their reimbursements.

And on Tuesday, UMC administrators will tell Clark County commissioners what treatments and programs they may need to drop because Medicaid payments don’t cover the hospital’s costs, and the hospital can’t afford to go in the hole.

Indeed, the Nevada State Medical Association said other pediatric specialists may also stop taking Medicaid patients because the government reimbursements don’t cover the cost of delivering the care.

Rare Treatment Is Reported to Cure AIDS Patient

Doctors in Berlin are reporting that they cured a man of AIDS by giving him transplanted blood stem cells from a person naturally resistant to the virus.

But while the case has novel medical implications, experts say it will be of little immediate use in treating AIDS. Top American researchers called the treatment unthinkable for the millions infected in Africa and impractical even for insured patients in top research hospitals.

“It’s very nice, and it’s not even surprising,” said Dr. Anthony S. Fauci, director of the National Institute of Allergy and Infectious Diseases. “But it’s just off the table of practicality.”

The patient, a 42-year-old American resident in Germany, also has leukemia, which justified the high risk of a stem-cell transplant. Such transplants require wiping out a patient’s immune system, including bone marrow, with radiation and drugs; 10 to 30 percent of those getting them die.

“Frankly, I’d rather take the medicine,” said Dr. Robert C. Gallo, director of the Institute of Human Virology at the University of Maryland School of Medicine, referring to antiretroviral drugs.

Half of Doctors Routinely Prescribe Placebos

Half of all American doctors responding to a nationwide survey say they regularly prescribe placebos to patients. The results trouble medical ethicists, who say more research is needed to determine whether doctors must deceive patients in order for placebos to work.

The study involved 679 internists and rheumatologists chosen randomly from a national list of such doctors. In response to three questions included as part of the larger survey, about half reported recommending placebos regularly. Surveys in Denmark, Israel, Britain, Sweden and New Zealand have found similar results.

The most common placebos the American doctors reported using were headache pills and vitamins, but a significant number also reported prescribing antibiotics and sedatives. Although these drugs, contrary to the usual definition of placebos, are not inert, doctors reported using them for their effect on patients’ psyches, not their bodies.

In most cases, doctors who recommended placebos described them to patients as “a medicine not typically used for your condition but might benefit you,” the survey found. Only 5 percent described the treatment to patients as “a placebo.”

The study is being published in BMJ, formerly The British Medical Journal. One of the authors, Franklin G. Miller, was among the medical ethicists who said they were troubled by the results.

“This is the doctor-patient relationship, and our expectations about being truthful about what’s going on and about getting informed consent should give us pause about deception,” said Dr. Miller, director of the research ethics program in the department of bioethics at the National Institutes of Health.

Kidneys Surgically Stolen from India’s Poor in Kidney Transplant Racket

MINNEAPOLIS (AP) – In a frustrating quirk in government policy, the most tightly controlled drugs – like painkilling narcotics prone to abuse – are the ones that most often elude environmental regulation when they become waste.

Federal narcotics regulators impose strict rules meant to keep controlled pharmaceuticals out of the wrong hands. Yet those rules also make these drugs nearly impossible to handle safely as waste, say hospital environmental administrators.

Many would like to send controlled substances to landfills or incinerators to keep them out of waterways as much as possible. Instead, they are nearly always dropped into sinks and toilets by hospitals, nursing homes and other long-term care facilities.

The problem is huge, because more than 365 medicines are controlled by the U.S. Drug Enforcement Administration – almost 12 percent of all prescriptions, the agency says. They include widely used narcotics, stimulants, depressants and steroids – drugs like codeine, morphine, oxycodone, diazepam (often sold as Valium) and methylphenidate (often sold as Ritalin).

Even so, the charity predicted the 50,000 expected donations over the next five years would prevent many deaths.

LORD Winston, the fertility expert and Labour peer, is to begin breeding genetically modified pigs in the hope of providing organs for transplant to humans, it was reported yesterday.

Scientists in London and California have begun experiments to find a solution to record waits for organ transplants. In Britain around 8,000 patients are on waiting lists.

“People needing a new heart or liver are waiting for someone else to die – usually a violent death in a traffic accident,” Lord Winston wrote in a Sunday newspaper.

Lord Winston, who heads the Institute of Reproductive and Developmental Biology at London’s Hammersmith Hospital, expects the technique to provide a solution to the shortage of organs within ten years.

The scientists are introducing human genes into the animals to reduce the chances of the organs being rejected by patients’ bodies.

CALGARY – A Calgary researcher is getting ready to publish a groundbreaking study that links a popular food additive to reduced growth in the brain cells of snails — work that could have major implications for children’s health.

Michelle Ah-Seng is a 17-year-old high school student from Cochrane, just west of Calgary. She’s also the lead researcher on a University of Calgary study that offers the first solid proof that high concentrations of MSG, an additive used to boost flavour in everything from fast food to canned soups, can stunt the growth of brain cells.

Ah-Seng is one of 22 Grade 11 students spending six weeks of their summer vacation in labs and clinics at the University of Calgary as part of the 2008 Heritage Youth Researcher Summer Program, funded by the Alberta Heritage Foundation for Medical Research.

Her project involved directly dosing brain cells culled from snails with a concentration of monosodium glutamate equal to what might commonly be found in human blood or cerebral spinal fluid after eating a meal containing the additive, such as a bag of chips. Not only did the MSG inhibit growth of the snail’s brain cells, it also limited communication between them. The implications for human health aren’t hard to infer.

“There’s no difference between a snail brain cell and a rat or a human brain cell, only that there are fewer of them and (they’re) larger,” said Naweed Syed, Ah-Seng’s supervisor and a neuroscientist with the Hotchkiss Brain Institute at the University of Calgary’s faculty of medicine.

France may have to reconsider its medical definition of death after a heart-attack victim came alive in the operating theatre as doctors were about to remove his organs for transplant.

The patient, whose identity has not been revealed, recovered after a long period in intensive care and is now able to walk and talk.

The 45-year-old man owes his life to the fact that surgeons authorised to remove organs for transplant operations were not immediately available. Under experimental rules adopted in France last year, to make more organ transplants possible, the man had already reached the point where he could be officially regarded as dead. Similar rules – allowing the removal of organs when a patient’s heart has stopped and fails to respond to prolonged massage – already apply in several other European countries, including Britain.

NY considers creating ‘organ-removal’ ambulance

Saving the living has always been the No. 1 priority for a New York City ambulance crew. But a select group of paramedics may soon have a different task altogether: saving the dead. The city is considering creating a special ambulance whose crew would rush to collect the newly deceased and preserve the body so that the organs might be taken for transplant.

The “rapid-organ-recovery ambulance,” still in the early planning stages, could raise a host of ethical questions and strike some families as ghoulish. But top medical officials in the Fire Department and Bellevue Hospital say it has the potential to save hundreds of lives.

Generally in the U.S., only people who die at hospitals are used as organ donors, because doctors are on hand with life-support machinery and other equipment to preserve the organs and remove them before they spoil. Surgeons have only a few critical hours before kidneys, livers and other body parts suffer damage that renders them unusable.

Surgeons give hope to blind with successful bionic eye operations

Surgeons have carried out the first operations in Britain using a pioneering “bionic eye” that could in future help to restore the sight of the blind.

Two successful operations to implant the artificial electronic device into the eyes of two blind patients were conducted last week at Moorfields Eye Hospital in London, it emerged today.

The device — the first of its kind in the world — incorporates a video camera and transmitter mounted on a pair of glasses.

This is linked to an artificial retina, which transmits moving images along the optic nerve to the brain, and enables a patient to discriminate rudimentary images of motion, light and dark.

The operations at Moorfields were conducted as part of an international clinical trial of the technology, known as the Argus II retinal implant, which has already proved successful in restoring rudimentary vision to blind patients with common causes of sight loss such as age-related macular degeneration (AMD) and retinitis pigmentosa.

Dr. B: Well, it’s very bizarre. As you know, I’m a regular doctor, an M.D. I had the honor and the privilege of being selected as one of the house doctors, and I had the extreme honor of being one of the last students of Sir Alexander Fleming, the discoverer of penicillin. I mention his name so that you know I was immersed in medical school and research. And some years later, I had to give two glasses of water to a person who was doubled up in abdominal pain from his disease, because I had no other medication to give him at that moment. And he was in excruciating pain, and water performed miraculous relief for him. It gave him relief — within three minutes his pain diminished, and within eight minutes it disappeared completely, whereas he was doubled up eight minutes before and he couldn’t even walk, he completely recovered from that situation. And he started beaming from ear to ear, very happy, asked me what happens if the pain comes back? I said, “Well, drink more water.” Then I decided to instruct him to drink two glasses of water every three hours. Which he did, and that was the end of his ulcer pains for the rest of the duration that he was with me.
